Clinic Hours
Phone and office hours are:
9:00 a.m.-4:30 p.m., Monday - Thursday
9:00 a.m.- 12:00 noon, Friday
We encourage you to call us with your
questions or concerns during clinic hours
when our staff has access to your medical
records. After hours, phone calls are
answered by our answering service.
Scheduling an Appointment

When making an appointment,
please have your medical insurance information
available.
Your 1st Visit
Plan to spend one to one and one-half hours for
the entire appointment. You will be completing
paperwork and meeting with the Respiratory
Therapist and Nursing staff prior to seeing your
provider. We ask that you bring a list of current
medications & dosage, medical records, and
current insurance card with you. Arriving
promptly will facilitate your visit.

Please call your pharmacy at least 3 days in advance of
needing your medications refilled. The pharmacy will
contact our office for the necessary information for your
refill.
Insurance
We accept most insurance carriers, however
we always recommend that you verify your
coverage. If a referral is needed, please
contact your primary clinic and /or insurance
carrier to authorize the visit and to be sure
you have coverage. With your authorization,
we will submit a claim to your insurance on
your behalf. You will receive a statement after your
insurance has paid, indicating any
balance you may owe.
Payment Policy
All accounts are due and payable within 30 days from
the date of billing. If, under special circumstances, you
are unable to pay your bill, please contact our Business
Office, ProSource Medical Services, at 763-561-5349,
ext. 27, to discuss payment arrangements. Co-payments
must be paid at the time of service.
